Follow-Up Appointments
A cancellation within-48 hours of appointment:
We at Medical Transformation Center value your time and effort when scheduling an appointment. We ask that you give us at least two business days/48-hours prior notice for appointment cancellation.
1st Time – $150 Cancellation Fee
2nd Time – Full charge of the appointment
*After the second cancellation within 48-hours of an appointment, all future appointments must be pre-paid.
A “No Show” for an appointment:
We reserve the right to charge a fee for not showing-up for an appointment.
1st Time – $150 charge
2nd Time – Full charge of the appointment
*After the second no show, all future appointments must be prepaid.
*Any exceptions are to be reviewed by management and will be determined on a case-by-case basis.

Please understand that in order to provide the optimal efficiency to our clients, it is important to make every effort to be punctual and give advance notice of any alteration in schedule.
Upon scheduling of your first appointment, we will request a credit card to be kept on file to assist with any charges. By doing so, you are giving us permission to charge this card for cancellation and no show fees, as stated above.
Missing more than one appointment or habitual cancellations may be cause for expulsion from the practice.
